Welcome to the Cambie Animal Hospital, your preferred veterinarian in Vancouver, BC. We look forward to treating you and your pets with the utmost care and attention. Prior to your first visit we want to help you prepare for what to expect. Here in our New Patient Center we have provided everything you need to get started. Read through our what to expect information, and access our online forms for your convenience. You can also check out the payment options we accept to prepare for your appointment.
What to Expect at Your Appointment
When you bring your pet for their first appointment, we will take time to get to know them and you as the pet owner. This will include a detailed history for the pet, as well as any medical history you have for the animal. If you have a vaccination record or other applicable information, please bring this along when you come in for your appointment.
Online Forms
To help ease your burden for your visit to our veterinarian in Vancouver, BC we have included our paperwork online. You can access the documents you need and print them out at your convenience. We recommend filling out the following documents, as applicable, prior to your appointment. This will save you time and hassle with having to complete them in-house.
If you are a new patient at the Cambie Animal Hospital please fill out the New Patient Registration form. You will need to fill out information for you as the pet’s owner and for your pet. If you are bringing more than one pet, we will need a New Patient Registration form for each pet.
If your pet is getting pet surgery we ask you to fill out the Surgical Information Packet. Also, add your initials to the last two pages.
Each of these documents can be accessed online using AdobeReader®, which can be accessed here. We ask that you print out the documents and fill them out in pen. Once completed you can bring these with you to your appointment. Optionally, you can fax or email a scanned copy to our office prior to your visit.
Payment Options
When you come to the Cambie Animal Hospital for an appointment, we require all payments for services rendered. To accommodate your payment needs, we take Visa and Mastercard credit cards, as well as debit and cash. Please note we require credit or debit card holders to sign in person when using these forms of payment.
If you need an alternative payment method we accept CareCredit, which is a credit-based financial program. We can help you apply for CareCredit here at our office, or you can call in advance and submit your CareCredit application.
