In accordance with Indiana State Law, pet medical records may not be disclosed without the client's written consent to "any person other than the client or other veterinarians involved in the care or treatment of the animal". To help us better serve you and your pet, please print the following medical record release and bring it with you to the office at your next visit so that we may continue to serve you better. With your signed release we will be able to deliver vaccine information and medical history per your request to Grooming Salons, Boarding Kennels, and other Veterinarians.
