1. Payment is expected when services are rendered. In order to focus on our patients' needs, customer service and minimizing costs, payment at time of service is preferred, however we do have billing options. We accept all forms of payment at the time of service.
2. We accept credit cards (Visa, Mastercard, Discover, American Express), checks, and cash. All cards must be signed by the owner of the card.

Special Online Payment Instructions
**Important** If the card you are paying with is not under the same name and/or address as your bill (this can be found in the upper left hand corner of your billing statement), please include the account name and address in the "shipping" section to avoid any delay in the proper account being credited.
We also accept credit card payments over the phone during regular business hours, personal checks, money orders and cash (in person only - please do not mail cash). All forms of payment are also accepted at time of service.
