Medication Refill Request

This form is to request refills of medications your pet is currently taking. Medication refills require your pet to have been seen by a doctor here within the last year, if it is a medication they take on a daily basis. Please allow 24 hours for order processing.

Please be aware that some prescriptions (including, but not limited to antibiotics, pain medications and ear medications) may require an examination of your pet prior to re-filling. This is to ensure we are dispensing the proper medication.

Other medications require periodic bloodwork to be done prior to dispensing to ensure that your pet is healthy enough to handle the potential side effects.

IMPORTANT: Required bloodwork and examinations are done at the doctor's discretion. Refill requests are not confirmed until you have received notification from a staff member.
