Welcome to the Forsyth Veterinary Hospital. We provide comprehensive animal health care in Winston Salem, NC. As your new veterinarian, we want to make your first visit as seamless as possible. Take a look at the following information. We hope this info can help you save time and energy when coming to our office for your appointment.

What to Expect

During your first visit, you will meet with one of the leading Winston Salem veterinarians, Ruth M. Gillis DVM. Dr. Gillis is also the owner of the Forsyth Veterinary Hospital. Depending on your pet’s current needs, we will most likely begin with a physical examination and health history of your pet. 

If you have any records, such as a pet vaccination history or list of allergies, please bring this to the appointment. We also need to know about any medications that your pet is currently taking, including vitamins, as well as the type of food and treats provided for your pet. 

If this is an emergency visit to provide urgent care services, we will likely need to draw blood and perform lab testing. We may also take digital x-rays if the pet has internal injuries or concerns. 

Online Forms Available

As a new patient, we have some forms that you will need to fill out. We require you to complete the following:

  • New Patient Registration Form
  • Surgical Information Packet, if applicable

An optional form is the Member Wellness Registration Form. In order to make it easier for you, we have provided these online forms here. You can download and print these forms for you to fill out at your convenience prior to your appointment. If you do not have a printer, it is a good idea to look at the forms. This way you can bring any necessary documentation with you to add information to the forms in our office.

Payment Options

We accept a variety of pet insurance options including Trupanion, Nationwide, and ASPCA coverage. If you have a different type of pet insurance, please contact our office to see if we currently accept this coverage. In addition, we take CareCredit to help you pay for veterinarian services. You can apply for CareCredit here online or in our office. With CareCredit, you receive a credit line along with regularly provided specials that allow you to extend your payments over three to six months without having to pay interest.
