We have online forms that you can print out and fill in ahead of time before your visit to our Laporte veterinary clinic. You may bring them with you at the time of your first appointment, or better yet, email or fax them to us ahead of time (970-407-7977). This allows us to get to know you and your pet and have the information entered into our computer even before you walk in the door, saving your valuable time at the appointment and allowing our doctors more time to devote to meeting you in person! (PLEASE DO NOT EMAIL ANY ITEMS AFTER 9 AM FOR USE THAT DAY-THE TECHS ONLY CHECK EMAILS BEFORE 9 AM)
