Below is a transcript of the agreement form found with the Doggie Day Camp Enrollment Form our clients can pick up from our receptionists, fill out online, or download at home, print, and drop off with our receptionists. As well as a final signature and date, we ask our clients to initial after each statement.
I, _____________________________, owner or authorized guardian of ___________________, release Forest Home Animal Clinic (FHAC), employees, and volunteers from any and all liabilities for injuries to myself, my dog, or any other property of mine. I understand that every dog reacts differently and that animals, by nature, are unpredictable.
I understand that dogs can sometimes receive minor injuries during play at day camp, and if any problems develop with my dog(s), the staff will contact me as soon as possible. If I, or an emergency contact, cannot be reached, my dog will be treated as deemed best by the staff, and I assume full financial responsibility for any and all expenses involved.
I understand that I am solely responsible for any harm caused by my dog(s), while they attend FHAC Doggie Day Camp.
I certify that my dog(s) is/are in good health and has/have not been ill with any communicable condition in the last 30 days. I further certify that my dog(s) has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog.
I authorize FHAC to use any photos taken of my pet during their visit online or in future publications.
I have read and understand the general information and policies of FHAC Doggie Day Camp.
As directed by the City of Franklin, I acknowledge that the ceiling in the lower level Doggie Day Camp area is below 7’ in some areas and will exercise caution.
