Please bring along copies of your pet’s records or have them faxed to our facility at (414) 425-4762. Usually a quick phone call requesting records to be faxed to our clinic is all that is needed. We are happy to call your previous clinic for you, but some facilities require owner’s authorization or require records to be picked up in person. Please let us know if you need assistance with this process. If you have an applicable free exam health certificate from a local humane society, bring it along.
As a safety precaution, all pets must be on a leash or in an appropriate carrier before entering the clinic, while in the waiting area, and inside the exam rooms. No pet may be left unattended.
For pets who are possibly contagious, aggressive, or prefer privacy, we have a separate entrance and exit on the south side of the clinic for everyone’s safety and comfort. Ask one of our team members for more information.
Our knowledgeable, passionate, and caring team takes pride in our commitment to the Veterinarian-Client-Patient Relationship (VCPR). This is the critical basis for interaction among veterinarians, clients, and patients for the health of your animal.
According to the American Veterinary Medical Association (AVMA), a VCPR requires:
- The veterinarian has assumed the responsibility for making clinical judgments regarding the health of the patient.
- The veterinarian has sufficient knowledge of the patient to initiate a preliminary diagnosis for the patient.
- The veterinarian verifies consent to treatment and provides oversight of results.
- Patient records are well maintained.
